adc藥物結(jié)構(gòu)
adc藥物也叫抗體偶聯(lián)藥物(antibody-drug conjugate,adc),是由靶向腫瘤特異性抗原抗原的單克隆抗體(antibody)與小分子毒素通過(guò)連接子偶聯(lián)組成。其兼具單抗藥物的高靶向性以及細(xì)胞毒素在腫瘤組織中高活性的雙重優(yōu)點(diǎn),可高效殺傷腫瘤細(xì)胞,較化療藥物副作用更低。adc藥物作用機(jī)理
adc的作用機(jī)制分為6步完成:1.adc分子進(jìn)入體內(nèi)后,可通過(guò)單克隆抗體的導(dǎo)向作用與腫瘤細(xì)胞表面的抗原(靶點(diǎn)蛋白)結(jié)合;2.adc-靶點(diǎn)復(fù)合物被細(xì)胞內(nèi)吞;3.adc在溶酶體中降解;4.釋放小分子化藥;5.微管或dna遭到破壞;6.靶細(xì)胞凋亡。圖1.adc藥物作用機(jī)理(來(lái)源于網(wǎng)絡(luò))
adc藥物靶點(diǎn)的選擇
靶抗原的選擇是adc藥物開(kāi)發(fā)成功的主要因素。靶點(diǎn)是 adc藥物主要識(shí)別腫瘤細(xì)胞和被內(nèi)吞的主要媒介,影響藥物的成藥性和競(jìng)爭(zhēng)格局。
理想的靶點(diǎn)應(yīng)具備以下特點(diǎn):1.靶點(diǎn)是腫瘤細(xì)胞特異性的,不會(huì)出現(xiàn)于健康組織細(xì)胞;2.抗原應(yīng)在配體存在下通過(guò)內(nèi)吞作用內(nèi)化,并應(yīng)可再循環(huán)至質(zhì)膜;3.抗原應(yīng)是不分泌型的,分泌型抗原可與體內(nèi)循環(huán)系統(tǒng)中的adc藥物或裸抗結(jié)合,從而導(dǎo)致與腫瘤細(xì)胞結(jié)合的 adc 藥物減少,影響藥物的療效和安全性。
adc藥物在研靶點(diǎn)
目前15款上市的adc藥物對(duì)應(yīng)11個(gè)靶點(diǎn),分別為cd33、cd30、her2、cd22、cd79b、nectin-4、bcma、egfr、cd19、tissue factor和frα。據(jù)統(tǒng)計(jì)在研的425款adc藥物中較為熱門的靶點(diǎn)包括her2、egfr、cldn18.2、trop2、c-met、cd19、psma、muc1、bcma和pdl1,大多數(shù)為驗(yàn)證成熟的靶點(diǎn)。表1.已上市的adc藥物靶點(diǎn)

還有一些靶點(diǎn)處于臨床研究階段。其中有些靶點(diǎn)為雙抗或多抗,如her2雙抗、pdl1/pdl2雙抗、cldn9/cldn6雙抗。
